MAIN FUNCTION: Responsible for the safe transportation of residents in the Adult Day Health Care
program, Social Adult Day Care, Active Day, Non-Emergency Medical Transports, Dialysis
Transports, Laundry Transports, Activities trips and for special transportation request approved by the
Transportation Team Leader or Manager.
DUTIES AND RESPONSIBILITIES:
Responsible for the operation of facility transportation vehicles which includes but not limited to cars, trucks and multi-passenger vans with a focus on safety.
All drivers report to the Transportation Team Leader
Maintains defensive driving at all times responding to all safety signs, traffic and road conditions.
Assists registrants/residents/family members, and other passengers in boarding and exiting the vehicle and assures safe destination and arrival.
Operates lifts and securement devices; as necessary.
Will dress neat for work and wear the required Community Wellness Partners shirt with casual pants.

Interprets driving directions to plan the most efficient route service available.
Follows specific regularly scheduled routes for Adult Day Health Care/Social Adult Day Care/Doctor Appointments and initiates route changes upon the admission of new registrants.
Provides transportation as necessary for special functions outside the facility and transports residents back and forth to appointments as requested.
Fuels vehicles, as necessary or assigned
Will clean both the exterior and interior of fleet vehicles as needed or assigned
Pick up and /or drop off vehicles to and from our repair shops as assigned
In accordance with facility policies responds immediately to accident or medical emergencies with proper notification to designated parties relative to the accident/incident.
Maintains accurate, up to date records as required.
All drivers will notify the Team Leader if additional coverage is needed when a driver may be running late for a transport
If a passenger is going to be late to their destination the Team leader will notify the appropriate Daycare Director or Unit Secretary
Notifies the Transportation Team Leader of any vehicle problems for immediate attention to ensure vehicle safety at all times.
Must possess a valid/clean New York state driver’s license in good standing meeting insurers’ requirements.
A NYSDMV drivers abstract is required
Will perform other duties as assigned by the Transportation Team Leader or Transportation Manager
EDUCATION: High School Graduate or GED
PHYSICAL DEMANDS: Stands, bends, drives and walks most of the day.
Must have good eye
sight to ensure safe navigation of fleet vehicles.
Will push wheelchairs and assist individuals entering
and exiting the vehicles
