Job Summary
Will provide ambulatory transportation for enrollees of Senior Network Health using Senior Network Health’s vehicles.
Individual will also be responsible for maintaining and cleaning the vehicle he/she is responsible for.
Core Job Responsibilities
Will be responsible for the safe and timely pick-up and delivery of SNH enrollees to their health-related and/or social appointments.
Will be responsible for providing door-to-door assistance for members to and from the SNH vehicles, and contact supervisor with any safe access issues.
Will maintain a clean driver’s license and obey all vehicle traffic safety regulations while driving a SNH vehicle.
Will be responsible for obtaining routine maintenance of the vehicle that they are assigned.
Maintenance will be paid for by SNH, LLC and may include but not be limited to routine oil changes, tire rotations, inspections, and alignments.
Will be responsible for the interior and exterior cleanliness of the vehicle that they are assigned.
Cleaning will be paid for by SNH, LLC and will be done on a minimum of once weekly or more frequently as needed
Will transport members on errands, which may include grocery shopping, post office and medical supply pick up.
Responsible for reporting in to supervisor any deviations from daily schedule of appointments.
Responsible for reporting to Supervisor any issues in regard to maintaining safety in members’ homes.
Attends committee and management meetings, as appropriate.
Resolves personnel concerns at the departmental level, utilizing the grievance process as required.
Education/Experience Requirements
Required:
High school diploma or GED
Preferred:
Experience in community based elder care and/or home healthcare
Licensure/Certification Requirements
Required:
Valid NYS Drivers license with exceptional driving record
