LPN Assistant Unit Manager
Must be motivated, organized, flexible and must know how to direct staff to achieve unit goals
Position Scope:
Under the direction of the Unit Manager, this position’s responsibilities with the Unit Manager, day to day clinical management and assume the duties of the Unit Manager in her absence.
Qualifications/Education/Experience:
Graduate of an accredited nursing curriculum.
Current NYS Licensed Practical Nurse (LPN).
Two (2) years of Geriatric Nursing
Long Term Care Supervisory experience preferred
Possess leadership skills and willingness to work harmoniously with interdisciplinary team
Responsibilities:
Provide, assist, and monitor other team members to provide high quality nursing care using a holistic and person centered approach.
Be sensitive to the changing and ongoing needs of residents, families, and other caregivers.
Monitor care delivered to ensure the plan of care is being upheld and remains appropriate to resident needs.
Ensures the resident kardex is up to date as per the plan of care.
Follows facility policy and procedure for clinical and infection control practices.
Liaise with medical and other members of the interdisciplinary team to ensure quality care.
Liaise with community services and medical consultants to ensure successful transition to and from LutheranCare.
Assists Unit Manager with the completion of all assessments as outlined per facility protocol; i.
e.
: admission/Re-admission assessments, Risk Assessments, Pain Assessments, MMSE evaluations, Cornell Depression Scale, Psychoactive medication justification forms
Provide support to the Unit Manager/Supervisor, bringing to their attention any matters they should be aware of e.
g.
, accident and incidents, change in resident condition, etc.
Contribute to the implementation and ongoing adherence to clinical protocols and standards of care.
Provide therapeutic treatments and procedures as ordered by medical staff e.
g.
, wound care, obtaining lab specimens, etc.
Participate in medical rounds, assisting as necessary, and follow through with new/changed orders.
Update the appropriate medical staff with change in resident condition, laboratory results, resident/family concerns, etc.
Update family/responsible party with change in resident conditions, incident and accidents, etc.
Follow up with acutely ill residents, residents with a recent incident, and those with change in condition on a daily basis to ensure improvement.
If decline is noted update medical and Unit Manager.
Participate in weekly wound rounds- providing treatment and follow up as needed.
Communicates and collaborates with other members of the interdisciplinary team to maintain quality care.
Participate in staff meetings.
Participate in quality assurance and improvement audits and projects as required.
Maintain medical records in accordance with facility policy.
Assist in maintaining unit equipment, supplies, and inventory.
Promptly report equipment/maintenance problems to relevant individuals.
Performs other related duties or miscellaneous duties as assigned within scope of practice.
