CABVI is seeking a (Vision) Care Coordinator IIto provide information on CABVI services and programs.
This position will reach out to consumers, market programs, complete intakes, develop service plans, coordinate therapy, process referrals, and provide services and rehabilitation therapy assistive services to enable consumers who are blind and visually impaired to achieve their highest level of functioning.
*Responsibilities Include:* * Respond to community inquiries regarding services for previously unserved (new) consumers.
Provide comprehensive accurate information on all therapy and programs available.
Complete insurance and medical network information, obtain signed releases.
Review privacy practices and confidentiality guidelines.
Secure medical records.
Assist with reopens and self-referrals as needed.
* Complete regular and ongoing follow up with consumers who have completed therapy.
Assess if additional services are needed.
Re-open those consumers for whom circumstances have changed.
Update releases, insurance, and medical records.
* Determine eligibility for funded programs which may include advocating with funding program staff.
Convey financial responsibility to the consumer.
* Develop rehabilitation plans for both new and reopened consumers and make internal therapy referrals.
Monitor timeliness of services, general progress, document therapy provided and report results.
* Assist consumers in applying for programs that are available to those with visual impairment such as the NY State Park Pass, NY State Veterans Annuity Benefit, Talking Books and others.
* Make internal referrals to agency departments or services, including but not limited to social work, orientation and mobility, vision rehabilitation therapy, occupational therapy, low vision, assistive technology and more.
* Make referrals to community programs when needs are identified such as, childcare, foodbank, HEAP applications and more.
* Work as a member of the Preschool Vision team.
Assist with scheduling the screenings.
Comminate professionally with community preschool staff.
Travel to the screening location.
Set up screening site.
Greet preschoolers and helping them to feel comfortable, conduct testing, record results and complete screening reports.
* Distribute marketing materials to community groups for promotion of special events.
Reach out to consumers to solicit special event program engagement.
Provide representation at any needed community events to promote services.
Work with the Rehabilitation Services Sales Manager to conduct community presentations to medical groups and community organizations as needed, participate as a presenter during tours of the VHWC.
* For patients who receive low vision exams at one of our low vision centers, follow up on equipment dispensed.
Read through exam notes, ensure devices are used successfully, review device care and usage, identify natural supports and work toward implementing assistance.
Schedule a return to clinic as needed.
* Provide VRT instruction, under the supervision of a VRT, in areas of communication, dispensing adaptive equipment as needed.
This equipment may include timepieces, calendars, and writing tools.
Mark appliances and other equipment to ease consumer usage, this would include instruction in use of markings.
Assist VRT’s with group instruction.
* Complete any vision rehab identified research, including soliciting research subjects, serving as the first line of contact for consumers, coordinate with the clinical team to administer study treatments as needed, ensure proper protocols are followed, collect and document data, process any research payments to consumers.
* Work with CABVI Low Vision Specialist to conduct tele-medicine exams.
Work as the remote low vision exam assistant operating the iPad, conducting testing under the direction and supervision of the specialist, gathering data, evaluating home, school and work environments, dispense recommended low vision equipment, providing information on usage and care.
* Work with assistive technology training team and low vision staff to dispense loaner CCTV’s and other purchased devices.
Transport devices to the home, school or workplace; install and provide instruction.
Work with consumers to use the device and complete tasks.
Provide usage and care guidelines, follow up after placement.
Complete any documentation required.
Assist with maintaining inventory and tracking loaners.
* Provide assistance with residential programs including but not limited to program promotion, participant enrollment, registration, securing medical records and signed releases.
* Provide cell phone instruction to consumers modifying adapting and accessing features that are useful.
* Develop weekly schedule to maximize efficiency and enter your schedule on Outlook.
Document mileage and services on TICR daily.
* Provide coverage and assistance to Rehabilitation Department Assistants when needed for front desk coverage.
Assist with low vision pre-evals as needed.
*Requirements Include:* * Bachelor’s degree required in human services, health services, education, or related field with a minimum of two years' experience in the Human service areas which may include research, health care and screenings, outreach or mental health.
* Bachelor's degree in marketing, communication, or psychology/sociology may also be considered with working experience in human services.
* Must meet NYSCB contractual requirements.
* Extensive travel required.
Instruction may occur in a variety of environments including consumer’s home or school, CABVI and/or the community.
* Must be flexible to meet the business demands and alter ones work schedule/work hours as needed.
* Must have ability to effectively present information and respond to questions from consumers, professionals, and the general public.
* Must have ability to persuade and encourage those in need to pursue services and programs.
* Must have ability to address correspondence and emails in a timely manner.
* Must have excellent communication, listening and organization skills.
Strong skillsets in areas of teamwork.
* Basic knowledge of computer systems and office software; Microsoft Office Suite, CIS, TICR, and more.
* Must have a good knowledge regarding low vision aids and adaptive equipment, use of an iPad, and electronic documentation.
* Must be willing to learn new technology.
* Must have the ability to define problems, collect data, establish facts, and draw valid conclusions independently.
* Must be able to recognize consumers status in relationship to their level of acceptance and respond accordingly.
