*_ *JOB DESCRIPTION*: The mission of The Neighborhood Center Outpatient Program is to treat, promote and empower individuals on their road to their recovery from the effects of substance use, mental health disorders, and/or other behavioral healthcare issues.
The Nurse Practitioner (NP) will provide medical and psychiatric evaluation, order routine lab work and diagnostic testing to medially assess the psychiatric and substance use disorder level of care needs for the patient.
The NP will also make appropriate medical and psychiatric referrals, assist staff in understanding the patients’ needs, with all work performed under the medical and clinical supervision of the Program’s Psychiatrist.
The Nurse Practitioner (NP) will work on a Full-Time basis at The Neighborhood Center Outpatient Program*.
* *SPECIFIC RESPONSIBILITIES INCLUDE:* * Delivers a high standard of patient care as Nurse Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.
* Performs routine or annual physical examinations and be a first point of contact for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan or treat complex, unstable, comorbid, episodic, or emergency conditions in collaboration with other health care providers as necessary.
* Consults with or refer patients to appropriate specialists when conditions exceed the scope of practice or expertise.
* Instigates necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
* Prescribes safe, effective and appropriate medication as defined by current legislative framework.
* Prioritizes health problems and intervene appropriately, including initiation of effective emergency care.
* Promotes health and well-being through the use of health promotion, health education, screening and therapeutic communication skills * Communicates clearly regarding the health status of the patient using appropriate terminology, technology and format.
* Maintains complete and detailed and contemporaneous records of patients' health care plans and prognoses within the Electronic Medical Record.
* Counsels patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over-the-counter (OTC) medications, or herbal remedies.
* Detects and responds to adverse drug reactions, with special attention to vulnerable populations such as infants, children, pregnant and lactating women, or older adults.
* Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research.
* Provides safe, evidence-based, cost-effective, individualized patient care.
* Participates in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained * Maintains current knowledge of state legal regulations for nurse practitioner practice including reimbursement of services.
* Ensures that patient centered care and safety is central to the culture, philosophy and organization of the practice.
* Maintains a professional manner and adheres to the policies and procedures of the Agency.
* Performs additional tasks as assigned *REQUIRED QUALIFICATIONS*: * Knowledge of and experience with professional nursing principles and techniques.
* Knowledge of commonly used medications, dosages and their physical and psychological effects and possible adverse reactions.
* Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and deformities including symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures.
* Possesses excellent interpersonal, communication and negotiation skills with ability to convey understanding of complex issues.
* Must work well with people of all social, economic, and cultural backgrounds.
* Demonstrated effective oral and written communication skills.
* Demonstrated strong organizational and time management skills including experience with preparing accurate/timely reports and records.
* *Current certification as a Nurse Practitioner (NP) and licensed in accordance with New York State law and current Drug Enforcement Administration (DEA) registration with no restrictions required and two (2) years of experience practicing as a Nurse Practitioner with one (1) year of that experience in a position providing direct services in an integrated care* setting preferred.
