RN Admission Nurse (Mon.-Fri.)

10am-6pm No weekends/No holidays The Grand Rehabilitation and Nursing at Rome is actively seeking an Admissions Nurse for our 160-bed Skilled Nursing Facility New York State Registered Nursing Licensing Required Excellent Compensation and Comprehensive Benefits Package provided! Duties Include: Initiate admission assessments on new Residents / hospital returns & initiates initial care plan.
Assist with the development & direction of the Nursing Department as directed by the DON.
Assess PRI's & coordinate all components of the admission process with the interdisciplinary team.
Responsible for auditing records to ascertain compliance with facility standards.
Perform required charting according to policy / procedure.
Complete MDS, care plans and other documents as assigned.
Maintain the confidentially of all Resident information as per HIPAA and Corporate guidelines.
May be assigned to supervise, direct and/or provide Resident care as necessary.
Requirements/Qualifications: Must have an active / unrestricted NYS Registered Nurse License Should have excellent clinical assessment skills Ability to make great independent decisions Computer savvy; proficient in MS Office (Excel, Word, Outlook) Excellent communication and interpersonal skills.
