RN Case Manager

Responsibilities: - Provide primary care to patients under the direction of a physician - Conduct patient assessments and develop individualized care plans - Collaborate with interdisciplinary healthcare team members to coordinate and manage patient care - Monitor and evaluate patient progress, adjusting care plans as needed - Ensure accurate and timely documentation of patient information in medical records - Facilitate discharge planning and coordinate post-discharge services - Utilize case management techniques to optimize patient outcomes - Implement utilization management strategies to ensure appropriate use of healthcare resources - Provide patient education and support to promote self-management and improve health outcomes - Adhere to HIPAA regulations and maintain patient confidentiality Responsible for supervision of LPNs, HHAs, PCAs, RAs Provide training for PCA program as well as certfication Qualifications: - Valid registered nurse (RN) license - Minimum of 2 years of nursing experience, preferably in a clinical or case management setting - Strong knowledge of medical terminology, disease processes, and treatment modalities - Proficient in conducting comprehensive patient assessments and developing care plans - Excellent communication and interpersonal skills to effectively collaborate with patients, families, and healthcare team members - Ability to prioritize tasks and manage time effectively in a fast-paced environment - Experience working with electronic medical records systems is preferred as well as paper medical records * If you are a dedicated registered nurse with a passion for providing high-quality patient care and have the skills necessary for case management, we encourage you to apply for this position.
