Job Summary
Under the supervision of the Registered Nurse/Therapy Care Manger and/or Supervisor, the HHA is responsible for providing personal care, activities of daily living, and HHA related tasks within their scope of duties for patients assigned to him/her.
Core Job Responsibilities
Accepts assignments and performs scope of duties within the Federal and New York State Rules and Regulations, seeking and obtaining sufficient information/training necessary to function effectively in the home health care setting.
Completes tasks that are directed by the Home Health Aide Care Plan as written by the Registered Professional Nurse and/or Therapist based of the evaluation of the patient’s needs and within the scope of duties of a HHA.
Strives for a safe environment for the patient; adheres to standard precautions.
Observes the patient for changes in condition and reports to the Register Nurse/Therapy Care Manager and/or Supervisor as indicated in the patient’s plan of care.
Participates in patient care conferences as appropriate.
Completes documentation per agency standards/policy utilizing an Electronic Medical Record (EMR) system (paper system back-up).
Maintains agency productivity standards.
Participates in regular team meetings and activities.
Participates in departmental and organizational committees and meetings as applicable.
Participates in the orientation of new personnel, mentors peers, and promotes collaborative teamwork.
Performs all duties and responsibilities in accordance as defined by Federal and New York State Rules and Regulations.
Maintains appropriate professional boundaries.
Maintains a working knowledge of, and adheres to applicable federal/state regulations including, but not limited to, patient confidentiality, release of information, and Health Insurance Portability and Accountability Act (HIPAA).
Interacts in a manner, which is professional, respectful, positive, helpful, and which promotes trust.
Attends agency in-services and educational programs, and maintains professional growth and development.
Participates in the quality improvement and utilization review activities of the organization.
Education/Experience Requirements
Required:
At least one year experience working as a Home Health Aide.
Licensure/Certification Requirements
Required:
Completion of a course study approved by the New York State Department of Education for a Certified Home Health Aide or a Certified Home Health Aide competency program.
Current certification to practice as a Home Health Aide in New York State
Valid New York State driver’s license.
