Job Summary
The Nurse Practitioner is responsible for providing healthcare to patients and families focusing on health maintenance, disease prevention, patient education and counseling.
Works in collaboration with a physician utilizing established guidelines as needed within established scope of practice.
Core Job Responsibilities
Performs physical exams on all patients, neonate through the geriatric population.
Obtains and documents patient's medical history.
Identifies health risk factors.
Diagnoses and treats common acute and chronic illnesses or injuries.
Orders and interprets diagnostic tests, i.
e.
, laboratory work, x-rays, EKG.
Prescribes medications per state laws.
Prescribes non-pharmacological therapies based on the patient's diagnoses, health history and age.
Encourages positive health behaviors for health maintenance and the prevention of disease through education and counseling.
Always applies standardized care guidelines in clinical practice.
Assesses and documents patient and/or family psychosocial issues.
Works with the patient and family to formulate a patient treatment and education plan.
Provides counseling and referrals to other healthcare professionals and community resources when appropriate.
Collaborates with physicians and other members of the health team to plan optimal care for the patient.
Responds appropriately in emergent situations
Provides follow-up care, determines effectiveness of the treatment plan, reassesses and changes the plan as needed.
Document all findings, interventions and results.
Maintains patient confidentiality at all times.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Provides age appropriate, culturally sensitive care to the assigned patient population.
Stays current with medical advancements, new technology, new drugs through continuing education classes, society meetings, professional journals etc.
Follows the five medication rights and reduces the potential for medication errors.
Treats patients and families with respect and dignity.
Attends all departmental, organizational and medical staff committee meetings as appropriate.
Perform other duties as required.
Education/Experience Requirements
REQUIRED
Masters Degree from an accredited Nurse Practitioner's Program or certification from an accredited NP program required.
Minimum 1 year of experience in a clinical setting.
Knowledge and competency in a wide range of clinical areas on the job.
Familiar with EHR/EMR software.
Licensure/Certification Requirements
REQUIRE:
Current NYS Registered Nurse License and current NYS Nurse Practitioner License.
Current BCLS Certification
Licenses or other definitive technical skills: Must provide a current New York State nursing license, nurse practitioner license and BCLS certification pre-employment and as required throughout employment.
PREFERRED:
Active ANCC certification in relevant specialty
