Job Summary
Provides nursing rehab care as well as direct and indirect registrant care; under the direction of a Registered Nurse.
Assists registrant with activities of daily living, providing for personal care and comfort.
Core Job Responsibilities
Provides direct restorative care and delegated formalized tasks as assigned; completing work accurately, safely, and in a timely manner.
Ambulates registrant in the hallway; using walkers, canes, crutches or tow persons to the distance specified with nursing assistance and or equipment indicated following the care of plan.
Observes and reports registrants ability to ambulate, distance of ambulation, and gait during ambulation to Patient Care Coordinator.
Provides group strengthening exercises – active and passive – range of motion four times a week with enthusiasm and motivates registrants to participate.
Assists registrants with ambulating, handling and care of clothing and other personal property including dentures, glasses, hearing aids and prosthetic devices.
Assists with bringing items to the Registrant’s designated locker.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors
Answers call system and anticipates registrant’s needs
Assists with removing food trays, cleans dishes, tables, and steam tables after meal time in a timely manner
Provides personal care; showers , nail care, and shaving as scheduled and as the need arises.
Encourage good grooming.
Assures linen is folded and placed in linen closet, and notified Secretary when supply order is not complete.
Changes bed with fresh lines as needed
Completes Certified Nursing Assistant requirements
Education/Experience Requirements
Required:
High School graduate or equivalent.
New York State Nurse Assistant Certification.
Two (2) years experience in a health care facility or community setting.
Ability to work independently as a member of a health team.
Knowledge and skills in basic nursing rehab, principles and practices.
Prior experience with the geriatric population
Licensure/Certification Requirements
Required:
New York State Nurse Assistant Certification.
