Job Summary
The Care Manager is a Registered Nurse who is responsible for the planning, coordination and supervision of all healthcare plan services for members for an assigned caseload.
Core Job Responsibilities
Collaborates with member, family, physician, social worker and service providers to insure appropriate member care for mutual achievement of healthcare plan goals and objectives.
Complies with all federal and state regulations including HIPAA.
Ability to effectively communicate in English, both verbally and in writing.
Additional languages preferred.
Ability to organize and implement plans, efficient and flexible in problem solving.
Able to motivate and educate members.
Proficient computer skills necessary.
Education/Experience Requirements
Required:
Three to four years of professional nursing
Demonstrates leadership abilities and functions well in interdisciplinary groups.
Preferred:
1 - 2 years of case management or homecare experience
Licensure/Certification Requirements
Required:
RN or BSN.
Current license from the NY State Board of Registered Nursing.
Current CPR certification
Valid state driver’s license and reliable automobile
