*JOB DESCRIPTION: *The Community Psychiatric Supports Treatment Counselor will provide professional services under the New York State Child and Family Treatment and Support Services (CFTSS) programming.
CFTSS are mental health services under New York State Children’s Medicaid provided at home or in the community.
All service are child centered, family focused, strength based, multisystem, and community based.
Services are delivered in the least restrictive environment for the child/family, in a trauma-informed, culturally and linguistically competent manner.
*SPECIFIC RESPONSIBILITIES INCLUDE:* * Assists child, youth, and family to achieve stability and functional improvement in daily living, personal recovery, and or resilience, family and interpersonal relationships, and school and community integration.
* Assists youth in home or community by helping improve communication with family, friends, and others for a child who may be at risk of or who has a mental health or substance use diagnosis.
* Assists with Crisis avoidance by restoring stability and functioning, preventing crisis episodes from occurring, or reducing intensity and duration of an active crisis.
* Provides Intensive intervention by assisting the individual with developing and implementing social, interpersonal, self-care, daily living, and independent living skills to restore stability to support functional gains and adapt to community living.
* Provides intermediate term crisis management - stabilizes child in the home and natural environment and assists with goal setting to focus on issues identified from mobile crisis or emergency room intervention and other referral sources.
* Provides Rehabilitative Psycho Education and Rehabilitative Supports.
* Adheres to the Standards of Care described in the Medicaid State Plan Children and Family Treatment and Support Services Provider Manual for Children’s Behavioral Health Early and Periodic Screening and Diagnostic Treatment (EPSDT) Services and any subsequent updates.
* Ensures adherence to the standards of any and all program funding bodies.
Ensure that accurate and timely records are kept.
* Refers families or individuals whose needs are not within the function of The Neighborhood Center, Inc.
CFTSS to other agency programs/services or to other appropriate CFTSS services in the community.
* Responsible for assessing the need for, and requesting the purchase of, supplies, materials, equipment, and repairs.
* · Maintains a work and program environment that is safe, healthy, orderly, and attractive.
* Ensures the fulfillment of training requirements of any funding and regulatory bodies.
* Prepares and maintains statistical records, program records and prepares reports as required.
* Maintains self in a professional manner at all times, adhering to Neighborhood Center policy, procedures and in keeping with the mission statement.
* Punctual and regular attendance is an essential responsibility of each employee.
Employees are expected to report to work as scheduled, on time and prepared to start working.
* Performs other responsibilities as assigned.
*REQUIRED QUALIFICATIONS:* * Travel to homes within the community is required.
* Must have clean and valid Driver’s License; access to reliable transportation.
* Able to work a flexible work schedule if necessary.
* Must reside in Oneida, Herkimer, Otsego, Chenango, Delaware, Schoharie Counties.
* Master’s Degree in Social Work Counseling, Psychology or related Human Services field plus one (1) year of applicable experience or who have been certified in an Evidenced Based Practice (in lieu of one year of experience requirement).
And/ other aspects of CPST services except for counseling may be performed by individuals with a minimum of Bachelor Degree and two (2) years of experience in children’s mental health, addiction, and/or foster care/child welfare/juvenile justice.
These include, Registered Professional Nurse with one (1) year of behavioral health experience; Licensed Occupational Therapists and Licensed Creative Arts Therapists to the extent they are operating under the scope of their license.
OR * Bachelor’s Degree and certification in an Evidenced Based Practice consistent with the CPST component being delivered (may provide certain services).
OR * Bachelor’s Degree and three (3) years applicable experience in children’s mental health, addiction and/or foster care/child welfare/juvenile justice or other related Human Services field and no certification in an Evidence Based Practice (may provide certain services).
